Registered Nurse Case Manager Requirements

• Current, unrestricted Registered Nurse (RN) license
• At least 1 year of professional nursing experience, preferably in case management, care coordination, utilization review, or discharge planning
• Strong knowledge of care transitions, discharge planning, and readmission prevention strategies
• Experience conducting patient assessments and identifying post-discharge needs
• Proficiency with electronic medical records; Epic experience is preferred
• Excellent communication, organization, critical-thinking, and collaboration skills
• Ability to work effectively with patients, families, physicians, nursing teams, community providers, and utilization review professionals

RN Case Management Responsibilities

• Collaborate with hospital-based physicians, patients, families, nursing teams, and utilization review partners to support continuity of care
• Complete initial face-to-face assessments for admitted patients within 24 hours to identify discharge needs and potential barriers
• Develop and coordinate individualized discharge and care transition plans
• Facilitate safe discharges in partnership with community providers and ambulatory care teams
• Help identify opportunities to reduce avoidable hospital readmissions
• Communicate patient care needs and transition plans clearly to the appropriate members of the care team
• Document assessments, interventions, follow-up plans, and care transitions accurately in the electronic medical record
• Monitor progress toward discharge goals and address obstacles throughout the patient stay
