Title: RN Case Manager - Home Health

Schedule:
- Shift: 8:00 AM - 4:30 PM

Assignment Details:
- Guaranteed Hours: 40
- Contract Weeks: 12

Requirements:
- 2+ years of home health experience
- Case management experience
- OASIS experience
- Wound care experience preferred

Roles & Responsibilities:
- Manage a caseload of 5-6 patients daily.
- Conduct assessments and develop care plans.
- Coordinate patient care and communicate effectively with interdisciplinary teams.
- Ensure compliance with documentation standards and regulations.
- Provide education and support to patients and families.
