Licensed Practical Nurse (LPN) – Night Shift Join YesCare as a Licensed Practical Nurse (LPN) and play a critical role in delivering compassionate and effective patient care within our dynamic healthcare program. Responsibilities Assist in assessing the health status of patients and contribute to the development and modification of individualized care plans. Provide safe, effective nursing care for multiple patients within the facility’s healthcare program. Implement patient education initiatives based on established teaching plans. Perform delegated nursing tasks consistent with educational preparation and within the scope of practice. Use a problem‑solving approach to make informed clinical decisions. Oversee onsite and offsite patient scheduling in collaboration with the care team. Ensure follow‑up care for patients, including telemedicine appointments and consult processes in accordance with policy and contract requirements. Participate in quality improvement initiatives and patient care committee meetings. Coordinate with the Regional Medical Director, site medical providers, Regional Nursing Director, and other site nursing leaders to ensure seamless patient care. Monitor CARES and ensure proper coordination as needed. Perform additional nursing duties as assigned to support patient outcomes and organizational goals. Qualifications Graduate from an accredited Licensed Practical Nursing program. Current licensure as a Practical Nurse (LPN) in the state of employment. Active CPR certification. Familiarity with state laws and regulations governing LPN practice. Successful completion of initial and annual clinical competencies to demonstrate required skills for the role.
