The Opportunity Wellness Equity Alliance is seeking an experienced and compassionate Field Operations Supervisor - Healthcare to provide day-to-day operational leadership for field-based teams delivering Street Medicine, Enhanced Care Management (ECM), and community health services. This role oversees service delivery across outreach sites, encampments, shelters, healthcare facilities, and other community settings. The Field Supervisor ensures that teams operate safely, efficiently, and in alignment with clinical, operational, regulatory, and partnership expectations. The Field Supervisor also plays an active role in community outreach and care coordination, helping connect individuals experiencing homelessness and other vulnerable populations to medical care, behavioral health services, infectious disease treatment, social supports, and other resources. This is an opportunity for a hands-on leader who can balance people leadership, field operations, community engagement, performance management, and compassionate service delivery. Key Responsibilities Field Operations & Performance Plan, coordinate, and supervise daily field operations and activities. Assign schedules, monitor staff performance, and adjust resources based on operational needs. Ensure adherence to established procedures, policies, safety standards, and program requirements. Maintain organized workflows and support consistent execution across field teams. Monitor service delivery, productivity, visit completion, documentation compliance, quality measures, and staffing utilization. Compile, analyze, and synthesize daily and weekly performance data. Present key findings, trends, operational risks, and improvement opportunities to senior leadership. Develop and monitor action plans to strengthen accountability and improve operational performance. Identify workflow inefficiencies and recommend process improvements that reduce errors, rework, and downtime. Team Leadership & Collaboration Provide direct supervision to Community Health Workers, outreach staff, and other field support team members. Lead, coach, support, and evaluate field team members. Support onboarding and training with an emphasis on trauma-informed and culturally responsive practices. Provide operational support to mobile teams conducting Street Medicine and community outreach. Ensure staff have the equipment, resources, information, and operational support needed to work safely and effectively. Monitor completion of assigned outreach, care coordination, and documentation responsibilities. Address operational concerns and elevate performance or personnel matters to appropriate leadership when necessary. Promote strong communication and collaboration among field staff, clinical teams, leadership, and other departments. Serve as a primary operational point of contact between field teams and management. Foster a positive, accountable, collaborative, and team-oriented work environment. Community Outreach & Engagement Conduct and support outreach activities among unhoused and underserved populations. Help identify individuals needing medical care, infectious disease services, behavioral health resources, and social supports. Build trusting relationships using trauma-informed, culturally responsive, patient-centered, and harm-reduction approaches. Conduct telephonic and face-to-face outreach to identify social determinants of health affecting patient wellbeing. Support outreach to individuals living with HIV, HCV, STIs, or other infectious diseases who are not currently engaged in care. Assist individuals with linkage or re-engagement into appropriate medical and community-based services. Provide peer navigation and community-based support to help individuals access and remain engaged in care. Care Coordination & Enhanced Care Management Help participants overcome barriers to accessing medical care, behavioral health services, and social resources. Coordinate appointments, referrals, follow-up services, and community-based resources. Connect members with transportation and other essential supports. Support medication adherence and ongoing treatment engagement. Advocate for members when coordinating with healthcare professionals and community partners. Coordinate with hospitals and care teams to support discharge planning and continuity of care. Accompany members to medical appointments when appropriate and consistent with program guidelines. Support patient tracking and panel management activities. Perform Enhanced Care Management responsibilities consistent with the Lead Care Manager function and participate as part of the member’s multidisciplinary care team. Grant-Funded Program Support Support implementation of grant-funded public health initiatives through outreach, peer navigation, and care coordination. Conduct outreach to individuals who have not linked to care following diagnosis or who have fallen out of care. Facilitate referrals and connections to primary care, specialty care, and treatment services. Help patients remain engaged in ongoing treatment and care. Support established program goals related to care engagement, treatment adherence, and long-term health outcomes. Contribute to performance goals and service delivery requirements established for grant‑funded programs. Quality, Compliance & Safety Ensure field activities meet organizational, customer, regulatory, and program quality standards. Monitor completion and quality of required documentation. Reinforce safety practices and situational awareness among field staff. Respond appropriately to field incidents and ensure required reporting is completed. Support de-escalation and crisis response when necessary. Maintain accurate records related to services performed, operational issues, resolutions, and improvement opportunities. Provide regular updates regarding team performance, field operations, and program status. Community & Partner Relationships Serve as an organizational liaison to hospitals, school districts, shelters, correctional facilities, community-based organizations, and other program partners. Foster collaborative relationships that support integrated care across clinical, educational, correctional, community, and street-based environments. Represent WEA at site meetings, community forums, public health initiatives, and partner meetings. Support effective communication between WEA field teams, clinical teams, leadership, and external partners. Requirements Qualifications Required Qualifications Bachelor’s degree. At least five years of progressive healthcare management experience supporting multi-site or multidisciplinary programs. Experience supporting healthcare or community-based service delivery in at least two of the following environments: Correctional facilities School-based clinics Street Medicine or mobile outreach programs Professional or lived experience working with people experiencing homelessness, youth, justice-involved populations, or other underserved communities. Understanding of the operational and regulatory considerations associated with community-based healthcare models. Demonstrated experience supervising and developing teams. Strong organizational, communication, problem-solving, and leadership skills. Ability to operate effectively in dynamic field environments and respond to changing operational needs. Willingness and ability to travel frequently throughout California. Ability to successfully complete required background checks, including clearances necessary for correctional and educational site access. Preferred Qualifications Bachelor’s degree in Healthcare Administration, Public Health, Social Services, Public Policy, or a related field. Experience in public health, mobile healthcare operations, Street Medicine, Enhanced Care Management, or correctional health services. Experience working with multidisciplinary clinical and community health teams. Experience supporting grant-funded healthcare or public health programs. Familiarity with performance metrics, operational reporting, and continuous improvement initiatives. Work Environment This is a field-based leadership position that requires regular travel to program and partner sites. Work may occur in traditional healthcare settings as well as outdoor, mobile, street-based, shelter, educational, correctional, and other community environments.
