GrapeTree Screening Interview for CNAs

Warm Introduction

"Hi, this is [ ] with Nextaff Healthcare. Is now still a good time to chat?

Great! Thank you for taking a few minutes to speak with me. I'm going to run through a quick screening to make sure we're a great fit for each other. This should take about 8 to 10 minutes. Sound good?"

1. Basic Qualifiers

• Age Requirement
  1. "Are you 18 years of age or older?"
     • Yes - continue
     • No - "I'm sorry, 18+ is required for employment and healthcare compliance, so we wouldn't be able to move forward at this time."

2. Work Authorization

• "This position is not open for sponsorship. Are you legally able to work in the United States without restrictions?"
  • Yes - continue
  • No - disqualifier

3. Background Check

• "Are you willing to complete a criminal background check if selected?"
  
  • Yes - continue
  • No - disqualifier
• "Is there anything that might come back on that, that you'd like to share with me now"

4. Experience + Credentials

• CNA Experience
  • "Do you have at least 6 months of CNA experience?"
    • Yes - continue
    • No - disqualifier
• Long-Term Care
  
  • "Have you worked in long term care before? If so, for how long?"
• CPR / BLS Certification
  
  • "Do you currently have an active CPR certification?
  • And do you have BLS as well? BLS is not required for CNAs, but some facilities prefer it, so it can open up more shift opportunities."
• Agency Experience
  
  • "Next we'd like to verify your agency experience. What agencies have you worked with in the past or currently work with?"
  • "Great! We are partnered with a couple of other agencies and will always give you visibility to those as well."

5. Scheduling Preferences

• Preferred Shift Types
  • "What shift types do you prefer? Days, evenings, overnights?"
• Hours Per Week
  • "How many hours a week are you ideally looking to work?"
• Start Date
  • "If everything goes smoothly, when are you looking to start picking up shifts?"
• Nextaff does your first month's schedule for you. We do have a minimum requirement of 2 shifts per week. What shifts and dates would you like to work for your first months schedule?

6. Onboarding Timeline

• "Part of our process includes an electronic employee packet. Are you able to complete your onboarding paperwork within the next 48 hours?"
  • Yes - continue
  • No - ask for details, but this may delay submission to GrapeTree

7. App Verification (Nextaff N> Gage)

• You should be receiving a link from a separate number generated by our system for access to download our app. Using that link will automatically sign you in and prevent you from being locked out of your account. Please let me know if you don't receive it within 5 minutes. Once you are logged in you'll want to navigate to the "Home" tab, then select "Onboarding", this is where I will upload requests for credentials and employee paperwork for you to complete. Please finish anything assigned to Onboarding within 48 hours and let me know if you have any questions along the way.

. W2 vs 1099 Explanation/Benefits

• "One thing that sets us apart is that you will be a W2 employee, not 1099.
  • This means:
    • You do not need your own LLC
    • You do not need your own liability insurance
    • You do not have to file quarterly taxes
    • Your tax withholding is handled just like a traditional employer
    • This gives you stability and protection compared to most gig-based agencies."
  • Our pay periods are Sunday through Saturday every week and our pay day is every Friday.
  • There will also be a form in your onboarding packet to fill out if you'd like to sign up for health insurance. Just send me a quick message if you have any questions about that.

8. Sense Communication

• "One last thing before we wrap up:
  We use a dedicated text line for all communication regarding scheduling, payroll reminders, updated policies, and for you to send us any questions, documents, or requests.
• I'm going to send you a message from our text line right now.

9. Wrap-Up

"Perfect, that's everything I need right now. Thank you so much for your time today. I'll send your credentialing packet to the app. You'll access this in onboarding under home tab. Please make sure to upload all the required documents and let me know once that's completed."
