$53 - $56 hourly

Responsibilities:
  • Coordinate and manage patient care plans, ensuring each individual receives tailored support that aligns with their unique needs.
  • Conduct comprehensive assessments to evaluate patients' physical, emotional, and spiritual well-being, fostering a holistic approach to care.
  • Collaborate with interdisciplinary teams to develop and implement effective care strategies, enhancing patient outcomes and family satisfaction.
  • Provide compassionate guidance and education to patients and families, empowering them to make informed decisions about their care journey.
  • Monitor and document patient progress, adjusting care plans as necessary to ensure optimal comfort and quality of life.
  • Facilitate seamless communication between patients, families, and healthcare providers, promoting transparency and trust.
  • Lead and mentor nursing staff, fostering a culture of continuous learning and professional development within the team.
Qualifications:
  • Experience in hospice or palliative care, demonstrating a deep understanding of patient-centered care.
  • Ability to develop and manage individualized care plans, ensuring each patient's unique needs are met with compassion.
  • Proven track record of collaborating effectively with interdisciplinary teams to deliver holistic care.
  • Strong communication skills to educate and empower patients and their families, fostering confidence and understanding.
  • Ability to conduct thorough assessments and adjust care plans proactively, always prioritizing patient well-being.
  • Experience in advocating for patients, ensuring their voices are heard and respected in all care decisions.
  • Proficiency in maintaining accurate and timely documentation to support seamless communication across teams.
