Come work with us! /strong/ppstrongResponsibilities:/strong/pulliAssist the Physical Therapist in implementing individualized treatment plans for patients, focusing on improving mobility, strength, and functional abilities./liliProvide hands-on therapeutic interventions, including exercises, modalities, and mobility training to help patients achieve their rehabilitation goals./liliMonitor patients progress and report any changes in their condition to the supervising Physical Therapist./liliEducate patients on safety techniques, and strategies to enhance recovery and independence in daily activities./liliMaintain accurate documentation of patient care, treatment sessions, and progress notes as required./liliCollaborate with other healthcare professionals to ensure comprehensive and coordinated patient care./li/ulpstrongQualifications:/strong/pulliCurrent state licensure or certification to practice as a Physical Therapist Assistant./liliPrevious experience in home health care is preferred, but not required./liliCPR/lili2 step TB test/liliStrong communication and interpersonal skills to effectively engage with patients./liliAbility to work independently and as part of a multidisciplinary team./liliCompassionate and patient-centered approach to care./li/ul/div
