Type: Field role Type of Hire: Direct Placement Hiring Decision: Made by client agency Available Openings: 2 Responsibilities Develop and oversee individualized care plans. Coordinate care with interdisciplinary teams. Educate patients and families on disease management. Monitor patient progress and adjust care plans as needed. Requirements Certification: Licensed Registered Nurse with no restrictions. Experience: Minimum 2 years in home health care. Skills: Excellent clinical and communication skills.
