Location: Fresh Meadows, NY (Onsite)

Salary: $54,000 - $56,400 annually

Schedule: Full-Time, 35 hours/week (Monday-Friday)

Hours: 9:00 AM - 5:00 PM or 8:00 AM - 4:00 PM (based on departmental needs)

Position Summary

We are seeking a compassionate and organized Bilingual Case Manager (Korean/English) to provide assessment, service coordination, advocacy, and counseling services to geriatric clients within the community. The ideal candidate will have experience working with older adults, strong communication skills, and a commitment to helping clients access community resources and benefits.

Key Responsibilities

• Conduct client intake, assessments, and ongoing case management services.
• Provide advocacy, crisis intervention, information and referral services.
• Perform in-home assessments and evaluate client needs.
• Assist clients with accessing community resources, benefits, and support services.
• Maintain accurate client records, documentation, and statistical reports.
• Coordinate care with supervisors, nurses, and other healthcare professionals as needed.
• Develop and maintain relationships with community agencies and service providers.
• Participate in training sessions, seminars, departmental meetings, and supervisory sessions.
• Keep supervisors informed of client needs, concerns, and service-related issues.
• Ensure compliance with HIPAA, OSHA, Compliance, IT Security Awareness, Sexual Harassment Prevention, and other mandatory training requirements.
• Support organizational goals through excellent customer service and professional conduct.
• Perform additional duties as assigned.

Qualifications

Required

• Bachelor's Degree in Social Work or a related field such as:
• Human Services
• Psychology
• Public Health
• Counseling
• Criminal Justice
• Sociology
• Behavioral Sciences
• Biology or other related disciplines
• Bilingual proficiency in Korean and English (spoken and written).
• Minimum of 1 year of related experience in case management, social services, healthcare, community outreach, or a similar field.
• Strong communication, interpersonal, and active listening skills.
• Proficiency with Microsoft Office Suite and general computer applications.
• Ability to multitask, work independently, and maintain attention to detail.
• Authorization to work in the United States without sponsorship.

Preferred

• Previous Case Management experience.
• Experience working with seniors, older adults, geriatric populations, or NORC (Naturally Occurring Retirement Communities) programs.

Working Environment

• Fully onsite position in Fresh Meadows, NY.
• Office-based environment with regular phone and computer usage.
• Local travel using public transportation may be required.

Interview Process

• Initial phone interview with Talent Acquisition.
• Second-round departmental interview (in-person).
• Candidates will have the opportunity to visit and view the office during the interview process.
