LPN - Acute Rehab Care | Licensed Practical Nurse

Join a dedicated healthcare team in the Midwest region as a Licensed Practical Nurse (LPN) specializing in acute rehabilitation. This role involves delivering direct patient care through the nursing process, demonstrating clinical expertise, and upholding the highest ethical standards in nursing practice.

Key Responsibilities
  • Adhere to established safety protocols including infection control, safe patient handling, and critical lab notifications to maintain a secure care environment.
  • Administer medications, perform nursing interventions, and provide education to patients and their families to support recovery and health outcomes.
  • Conduct ongoing patient assessments to monitor status, enhance quality of care, and communicate findings effectively to the healthcare team.
  • Collaborate closely with supervising RNs or shift managers to ensure comprehensive patient care.
  • Document all care activities and quality measures accurately to support clinical decisions and compliance.
  • Delegate tasks appropriately while fostering teamwork and accountability within the care environment.
  • Engage with patients and families promptly to improve their experience and understanding of care plans.
  • Apply critical thinking in nursing practice and pursue continuing education to enhance skills and knowledge.
Education

Graduation from an accredited practical nursing program is required.

Experience

At least one year of practical nursing experience is preferred.

Licenses and Certifications
  • Current LPN license valid in the state or through compact licensure.
  • Graduate Practical Nurses must obtain licensure within 90 days of graduation.
  • American Heart Association Basic Life Support (BLS) certification required.

Physical and Mental Requirements

This position requires considerable mental focus and physical activity including lifting up to 35 pounds, standing, walking, and various movements such as pushing, pulling, and reaching. Frequent exposure to infectious and communicable diseases is expected, and occasional encounters with hostile individuals may occur.
