About the Position

• Case Management: Conduct home visits to manage an assigned adult/geriatric patient caseload (typically 5 visits per day or ~5.25 productivity points).

• Clinical Assessments: Perform admissions (Start of Care), Recertifications, Resumptions of Care, and Routine visits.

• Skilled Care: Administer wound care/wound vacs, manage IV therapy (PICC lines), and monitor chronic conditions (CHF, COPD) to prevent rehospitalization.

• Documentation: Complete accurate electronic charting using specialized home health software within mandatory compliance windows.

Requirements

• License: Active, unrestricted California Registered Nurse (RN) license.

• Experience: Minimum 1 to 2 years of recent, dedicated home health experience.

• Software Skill: High proficiency with electronic medical records (Oasis and HomeCare HomeBase / HCHB strongly preferred).

• Logistics: Valid U.S. driver’s license, reliable personal vehicle, and active auto insurance.

• Life Support: Active AHA BLS certification.
