Hospice Licensed Practical Nurse

Liberty Health seeks a compassionate Hospice Licensed Practical Nurse to provide person-centered end-of-life care in patients’ homes and community settings. The LPN will deliver skilled nursing under RN/physician direction, including assessments, pain and symptom management, medication administration, and wound care. Responsibilities include implementing care plans, documenting in the EMR, educating patients and families, and collaborating with an interdisciplinary hospice team. This role supports evidence-based, outcomes-focused care and requires strong communication, reliability, and respect for diverse backgrounds and end-of-life preferences.
