About the Opportunity

In this role, you will oversee the comprehensive health and well-being of a select patient panel while working within a highly collaborative, team-based care model. Physicians partner closely with Registered Nurses and Patient Care Administrators to proactively manage patient health and deliver an exceptional patient experience.
