Nurse Practitioner or Physician Assistant Primary Care

A community health center in Kansas City is hiring a Nurse Practitioner or Physician Assistant to provide primary care using the Patient Centered Medical Home model. Key Responsibilities. Provide diagnostic and therapeutic services, including history and physical exams, prescribing, and management of acute, chronic, and preventive care under a collaborating physician. Make referrals for hospital and specialty services where appropriate. Review and interpret lab, radiology, and consultation reports promptly, directing nursing staff or contacting patients directly with results and treatment recommendations. Deliver care aligned with current evidence-based guidelines and state, local, and federal requirements. Provide ongoing patient education on diagnosis and plan of care. Help establish standards of care in collaboration with other providers and staff. Document thoroughly — reason for visit, medical history, current medications, plan of care, orders, referrals, transitions of care, and follow-up. Identify barriers to care and coordinate referrals to internal support services and outside collaborating organizations. Collaborate with the Chief Medical Officer, Chief Clinical Officer, and other providers to evaluate services and develop programs. Participate in monthly Medical Executive meetings, committee meetings, quality improvement activities, peer review, and pharmacy and therapeutics. Stay current with advances in therapeutic intervention through ongoing literature review. Maintain continuing education requirements and patient confidentiality in accordance with HIPAA. Qualifications. Active, unrestricted Missouri FNP or PA license, in hand at time of application — this is a hard requirement, and license-eligible or in-process candidates will not be considered. Graduate of an accredited Nurse Practitioner or Physician Assistant program. Basic Life Support (BLS) certification. Active Missouri DEA and BNDD licensure preferred. Interest in chronic disease management and pediatric care. Commitment to working within a Patient Centered Medical Home model. Collaborative approach, with willingness to participate in committee and quality improvement work. Spanish language proficiency preferred. Compensation and Benefits. $104,536–$130,670 annually.
