Position: Wraparound Facilitator Location: Any HPMHC office location (Hays, Norton, Phillipsburg, Colby, Goodland, Osborne) Pay: $22.36 per hour | $500 SIGN-ON BONUS! Job Type: Full-Time | In-Person Schedule: typical hours Monday- Friday 8am-5pm Why You Should Apply? As a Wraparound Facilitator, you'll be the person who helps bring everything together. You’ll sit at kitchen tables, attend team meetings, connect families with resources, celebrate victories both big and small, and help children build the support systems they need to succeed. You’ll work alongside parents, caregivers, schools, providers, and community partners to create a plan that gives kids the best chance to thrive. Responsibilities Include: Facilitating Wraparound planning meetings with families and support teams Helping families navigate the SED/HCBS Waiver and PRTF Diversion Grant programs Coordinating services and community resources to support treatment goals Developing individualized plans that focus on strengths, needs, and long-term success Collaborating with families, educators, providers, and community partners Monitoring progress and adjusting plans to ensure positive outcomes Advocating for children and families within schools, agencies, and community systems Completing documentation and maintaining compliance standards Traveling within the service area to meet with families and community partners No two days are exactly alike—and every day brings an opportunity to make a meaningful difference. We accept equivalent work experience in human services (teaching, daycare, CNA, etc.) or a BA/BS degree. One year of related work = one year of college. Great with people — strong communication and empathy are a must. Experience working in human services, behavioral health, social services, education, or a related field preferred. Comfortable in a variety of human service settings. Must be able to work effectively with children experiencing Severe Emotional Disturbance (SED) and their families.
