This position requires flexibility to handle a mix of shifts including weekdays, on-call, and weekends. You'll work closely with patients in their home environment, focusing on health maintenance, disease prevention, and case management.

Key Responsibilities of the Registered Nurse - Home Health

• Observe, assess, plan, implement and evaluate nursing support for individuals
• Collaborate with health professionals/providers on health programs and services
• Coordinate communications and actions across health disciplines
• Order, review, and monitor prescribed medications
• Ensure medication policies are reviewed annually and revised as necessary
• Report client's condition changes to the physician
• Transcribe physician orders and related reports
• Document individual health statuses in nurse's notes
• Supervise licensed practical/vocational nurses as necessary
• Schedule and complete appointments for health-related issues
• Provide new employee and annual recertification training
• Train unlicensed staff on individual-specific health needs

Requirements of the Registered Nurse - Home Health

• Current WV RN License
• Experience in Home Health
• Proficiency in HCHB and OASIS
