Clinical Intervention Specialist Older Adult & Medically Fragile SOS Team

Position Summary The Clinical Intervention Specialist is responsible for providing consultation and training to outreach professionals as well as interventions as part of a multidisciplinary team that supports older adults and medically fragile individuals who have been, or are currently experiencing, homelessness. Essential Position Functions Clinical Intervention Specialist will provide consultation to staff on outreach teams working throughout NYC, recommending and training on specialized interventions, such as conducting relevant assessments, making referrals to appropriate community-based services such as homecare and socialization programs, completing housing applications, advocating within systems for benefits and entitlements, coordinating care and services, assisting with community navigation, such as accompanying to behavioral health and medical appointments. Facilitate and lead case-specific conferences in the community, including inpatient and outpatient behavioral health facilities, community-based social service provider sites, participants’ homes, shelters, and unhoused individuals living on the street. Train staff and provide some direct skill building to promote optimal mental and emotional health, including preventative strategies to reduce medical risk. Engage individuals around issues of substance use, mental health, medical concerns, family and social relationships, stress and symptom management, and medication management. Active participant in multidisciplinary meetings. Willingness to travel throughout Manhattan, Brooklyn, Bronx and Queens.
