Location: Remote + limited travel
Salary: $140,000–$180,000 Base + 15% Bonus

SUMMARY
Our client is seeking a Director of 340B Pharmacy Operations to lead the implementation and execution of 340B programs across a growing infusion business.

This is a hands-on leadership role for someone who understands 340B beyond compliance and can independently take a program or operational concept from idea through implementation, go-live, and ongoing optimization. The strongest candidates will bring direct 340B experience within home infusion or health-system/outpatient infusion environments.

WHAT YOU'LL DO
• Lead 340B implementations from planning through go-live and post-launch optimization, with a focus on infusion programs.
• Independently build and manage project plans, timelines, deliverables, dependencies, and implementation schedules.
• Support 340B operations across home infusion, outpatient infusion, mixed-use, and other pharmacy environments.
• Establish and optimize workflows around patient and site eligibility, accumulators, charge capture, billing, NDC mapping, and drug purchasing.
• Coordinate implementation activities across TPAs, wholesalers, pharmacy teams, health systems, finance, IT, and other partners.
• Manage TPA and split-billing setup, data feeds/EDI, account configuration, testing, and post-go-live auditing.
• Identify operational gaps, compliance risks, missed capture, and revenue opportunities, then develop and implement solutions.
• Develop SOPs, internal controls, audit processes, and corrective action plans.
• Support HRSA/OPAIS requirements, registrations, recertifications, audits, and ongoing program compliance.
• Help build the infrastructure and processes needed to scale the organization's 340B offering.

WHAT YOU BRING
• Direct 340B experience within home infusion.
• Health-system or outpatient infusion 340B experience.
• Specialty pharmacy 340B experience with infusion and strong implementation experience.
• Experience personally implementing 340B programs and operational changes rather than simply monitoring an established program.
• Demonstrated ownership of implementations, launches, conversions, or major operational projects.
• Experience with TPAs, split-billing platforms, wholesalers, accumulators, eligibility, charge capture, billing, and audit workflows.
• Working knowledge of HRSA/OPAIS requirements and 340B compliance.

OVERVIEW
This role offers significant long-term growth potential. The person joining will initially partner closely with pharmacy and infusion leadership to execute and scale the 340B strategy, with the goal of developing into the leader who can independently run the 340B program over the next 2–3 years.
