Description

American Traveler is seeking an experienced RN for a Care Coordinator position focused on cardiac and heart transplant patient management, requiring a CA RN license and ambulatory care experience.

Details

• Ambulatory care setting within a large academic medical center
• Care Coordinator unit focused on heart transplant and heart failure patients
• Patient population includes heart transplant recipients and patients with heart failure
• Responsibilities include immunosuppression monitoring, prophylactic antibiotic management, and monitoring for organ rejection and infection
• Day shift schedule: 4x10-hour shifts, 7:30 AM – 6:00 PM
• Floating to other facilities within the same health system is required

Requirements

• Active CA RN license required
• Current AHA BLS certification required
• Minimum 1 year of ambulatory care nursing experience required
• Cardiac nursing experience with management of heart failure and/or heart transplant recipients required
• Experience with Phoenix EMR preferred
• Experience managing other organ transplant recipients preferred
• Working knowledge of immunosuppressive therapies preferred
