RN Case Manager

CarePartners, a mission-driven health insurance company dedicated to coordinated care for Connecticut residents, is seeking a Registered Nurse Case Manager for our PACE Home Care program. In this role, you will manage a panel of participants, conduct in-home assessments, develop and coordinate individualized care plans, and collaborate closely with an interdisciplinary team to optimize outcomes. You will educate patients and families, ensure smooth transitions of care, and connect participants with community resources.
