In today’s ever-changing healthcare marketplace, HDSW offers person-centered, trauma-informed, culturally competent services to over 3000 people each year, through a unique blend of innovative inter-related programs. Position Overview: To assist clients living in the community to link to necessary medical, behavioral health, housing, social, employment, educational, and other services essential to meeting basic life needs. Sign-on Bonus: $3000 paid out over the first year of satisfactory employment.
