SummaryThe Bilingual Spanish family Coordinator for the School-based Early Support Program plays a pivotal role in providing comprehensive support and intervention services to children and families within the school setting. This position requires a compassionate and organized individual with a strong understanding of child development family dynamics and community resources. The Family Coordinator will work closely with school staff families and community partners to assess needs develop individualized plans and coordinate services to ensure the well-being and academic success of Functions:Assessment and Planning:Conduct thorough assessments of children and families to identify strengths needs and with school personnel and other stakeholders to gather relevant information and develop comprehensive support evidence-based practices and assessment tools to inform decision-making and goal Coordination:Coordinate and facilitate access to a range of support services including educational mental health medical and social as a liaison between the school families and external service providers to ensure seamless communication and coordination of service delivery and progress towards goals adjusting plans as needed to address evolving and Empowerment:Advocate for the needs and rights of children and families within the school and larger families to navigate systems and access resources by providing information education and family engagement in decision-making processes and encourage self-advocacy and and Reporting:Maintain accurate and up-to-date case records progress notes and documentation in accordance with program requirements and confidentiality and submit timely reports to document outcomes achievements and challenges encountered in service compliance with all relevant policies procedures and regulations governing case management and and Professional Development:Collaborate with interdisciplinary teams including school personnel social workers counselors and community agencies to enhance support services and achieve positive in ongoing training and professional development activities to enhance knowledge and skills related to child development family systems trauma-informed care and cultural in reflective practice and seek supervision and support as needed to promote personal and professional other related duties as Responsibilities:NoneMinimum Qualifications:Bachelors Degree in Social Work Psychology or Social Spanish At least 2 years experience in related interpersonal skills ability to work with diverse social economic and cultural in Microsoft to plan and carry out assignments to prioritize adhere to timelines and Behavioral Expectations:Maintain regular consistent and professional attendance punctuality personal appearance and adherence to relevant health & safety and successful performance including meeting productivity an understanding of the principles methods and processes needed to perform the job.
