Speech-Language Pathologist in Skilled Nursing Facility
Assess, diagnose, and treat residents with communication, cognitive-linguistic, and swallowing disorders. Develop and implement individualized treatment plans to promote residents' independence, safety, and quality of life. Conduct comprehensive evaluations and interpret findings to guide therapy. Deliver direct therapy services to improve communication, cognition, and swallowing function. Perform and interpret swallow assessments; recommend appropriate diet modifications. Document assessments, treatments, progress, and discharge in compliance with regulations. Educate residents, families, and staff on therapy plans and safe swallowing strategies. Collaborate with interdisciplinary care team and participate in care planning meetings. Monitor resident progress and adjust treatment plans as needed for optimal outcomes. Ensure compliance with professional, legal, and regulatory standards. Supervise Clinical Fellows and/or Speech-Language Pathology Assistants, if applicable. Work in a skilled nursing/long-term care facility with a collaborative, team-focused environment. May require assisting residents with complex medical needs and standing for extended periods.
