Job Title: RN Home Health Profession: RN Home Health Specialty: RN Home Health Duration: 13 weeks Shift: Monday to Friday Hours per Shift: 8:00 AM - 4:30 PM Experience: Minimum years of experience required License: Valid RN license Certifications: BLS Must-Have: RN case management experience Description: Under the direction of the Clinical Team Manager and within established organizational and departmental policies and procedures, responsible for providing skilled care and case management to patients and families. The RN case manager provides services utilizing nursing theory and processes. Communicates and documents observations and assessments. Promotes and maintains the health of individuals through direct care, teaching, counseling, and appropriate rehabilitative measures.
