Wellbound Certified Home Health Agency is seeking a Full-Time Home Care Educator to represent and educate our Case managers in the field.
THIS POSITION REQUIRES TRAVELING
Please note that you be required to travel locally.
Responsibilities:
Work hand in hand with Case Managers in identifying problems and areas for improvement.
Go to buildings to help identify problems with field clinicians.
Educate and work with clinicians to correct their documentation.
Review Patient charts.
Review and complete orders.
Requirements:
Valid NYS RN License.
5+ years of Home Care experience ( Required)
Knowledge in home care regulations- including Medicare and NYS regulations
Case Management experience (Preferred)
Clinical Experience (Preferred)
