Family Nurse Practitioner

Family Nurse Practitioner Location: TX – Fort Worth, 1650 W Rosedale St, Suite 220. Clinic Hours: Monday – Friday; 8am – 6pm. Schedule: Full-time, On-site. This role requires the practitioner to work from the posted location full time to enable consistent face-to-face collaboration and direct engagement. Candidates not within a reasonable commuting distance will not be considered unless accommodation is granted as required by law. Responsibilities Administer ordered therapies according to prescriber order, facility protocols, INS standards, and company policy. Complete admission process as directed by the Center Director. Perform physical assessment on each patient as warranted and for new consults. Document all pertinent data in the patient’s medical record. Report all significant changes or observations to the Center Director and follow up on any identified problems. Communicate effectively with other members of the IV team about patient status and observations. Maintain patient confidentiality by adhering to HIPAA laws and regulations. Evaluate the patient’s response to therapy and document findings in the medical record. Educate patients and/or family members regarding therapy plans. Participate in the Quality Improvement program. Ensure all medication orders are complete, appropriate, accurate, and up-to-date before treatment. Provide clinical coverage for nurse practitioners on PTO/LOA and may support new market launches. May travel to other Paragon clinics to provide coverage as needed. Minimum Qualifications Master of Science in Nursing and a minimum of two years of nursing experience; or an equivalent combination of education and experience. Current, active, valid, and unrestricted NP license for the applicable state. Active, current, and valid Family Nurse Practitioner Certification. Experience with IV insertion and maintenance required. Multi-state licensure required if providing services in multiple states. Satisfactory completion of a tuberculosis test and hepatitis B vaccine or signed waiver. Preferred Skills and Experience Credentialing with major payors, including Medicare and Medicaid, preferred. Experience with PICC and peripheral lines. Infusion reaction management experience. Titration experience. Medication mixing and infusion experience.
