Conduct individual and group sessions, coordination of treatment planning including: assist with referral and collateral contacts, case management and completion of biopsychosocial assessment. Must have a working knowledge of ASAM levels of care and the DSM-5 criteria for substance use disorders.

DUTIES AND RESPONSIBILITIES

Upon admission, obtain history and presenting information as well as orient new patient to program process and procedures. Provide patient with daily individual and group counseling, crisis intervention, as well as education and skills training. Collaboratively develops and updates an individualized treatment plan with the patient. Define all relevant community supports affecting treatment patient care including attorneys, family members, and other collateral agencies. Obtain signed releases of information to communicate with community supports. Update referring sources and family members on patient progress, if approved by the patient. Utilize patient-centered evidence-based interventions and rapport-building practices, including motivational interviewing techniques to enhance patient insight regarding diagnoses and patterns of use. Document treatment episodes, contacts, treatment plans and sessions according to regulatory compliance requirements. Coordinate comprehensive discharge plan to ensure seamless community reintegration. QUALIFICATIONS

Bachelor’s Degree and/or 2 years’ of addiction medicine or behavioral health experience serving culturally diverse populations. Preferred Requirements Master’s Degree in human services, social work, psychology, mental health, addiction medicine or related field.
