Behavioral Health Case Manager: Care & Discharge

Wooded Glen Recovery Center in Henryville, IN is seeking a Case Manager to support multiple units by coordinating care and discharge planning as part of the treatment team. You will maintain client records, communicate with referral sources, and ensure services on the individualized service plan are implemented.

The role requires a High School Diploma or equivalent, with a preferred Bachelor’s in Social Work or related field and experience in a behavioral health setting.
