LPN License Practical Nurse

LPN Charge Nurse – Full-Time

Cedar Hills Center for Nursing and Rehabilitation is seeking a dedicated LPN Charge Nurse to provide leadership and direct clinical care within a skilled nursing environment. In this role, you will support resident care delivery, guide nursing assistants, and collaborate with the interdisciplinary team to ensure high standards of safety, quality, and resident-centered outcomes. This position is ideal for a nurse who is confident in clinical decision-making and values both hands-on care and team leadership.

Shifts Available:

• 7:00 AM – 7:00 PM
• 7:00 PM – 7:00 AM License Required: Active LPN License (NC)

Key Responsibilities:

• Administer medications and monitor residents for therapeutic response and side effects
• Perform ongoing assessment of resident condition, including vital signs and clinical status changes
• Provide wound care, catheter care, and specimen collection as needed
• Oversee and support CNAs to ensure consistent, high-quality resident care
• Communicate changes in resident condition to the RN and interdisciplinary care team
• Participate in care planning, updates, and resident care coordination
• Maintain accurate, timely, and compliant clinical documentation
• Promote resident safety, comfort, dignity, and overall well-being
• Ensure adherence to facility policies, procedures, and regulatory standards

Qualifications:

• Active Licensed Practical Nurse (LPN) license in North Carolina
• Current CPR and First Aid certification
• Strong clinical judgment and ability to respond to changes in patient condition
• Leadership ability with strong communication and teamwork skills
• Compassionate, dependable, and resident-focused approach to care
• Ability to work effectively in a fast-paced skilled nursing environment
