\n Are you a compassionate, experienced RN who loves making personal connections and helping patients understand their options at a vulnerable time? As a Hospice Admissions Nurse, you’ll be the face of our care—helping families say “yes” to compassionate end-of-life support when it matters most.

What You'll Do as a Hospice RN Admissions Nurse:
\n
\n • Report directly to the Admissions Manager and serve as a key ambassador for our hospice services
\n • Visit patients and families in their homes, hospitals, or facilities to assess eligibility for hospice care
\n • Explain hospice philosophy and services clearly, compassionately, and with clinical confidence
\n • Guide patients and their loved ones through the admissions process, ensuring a smooth and supportive start to care
\n • Collaborate with referring physicians, case managers, and hospital or facility staff to coordinate transitions of care
\n • Use the nursing process to assess patient needs, support care planning, and document clinical findings
\n • Build trust and provide emotional reassurance to patients and families as they make this important decision
\n • Accurately complete required documentation and ensure compliance with state and federal regulations
\n • Work closely with the clinical and interdisciplinary team to ensure timely admissions and quality care handoff
Qualifications - What You’ll Bring:
\n\n
• Active RN license in the state of employment (or eligible to obtain)
\n • 1+ year of general nursing experience
\n • A hospice heart: compassionate, empathetic, and deeply committed to patient-centered care
\n • Strong assessment, communication, and interpersonal skills
\n • Comfortable visiting patients in hospitals, skilled nursing facilities, or home settings
\n • Valid driver’s license, auto insurance, and reliable transportation
\n • Current CPR certification
\n

Preferred Experience (Not Required):
\n\n
• 2+ years of hospice, home health, healthcare admissions, or medical sales
\n • Bachelor’s Degree and/or Certified Hospice and Palliative Nurse (CHPN) certification
\n • Background in case management, care coordination, or healthcare outreach
\n • Experience with electronic medical records and compliance documentation
