Case Manager

Cardinal Hill Rehabilitation Hospital, part of Encompass Health, seeks a Case Manager to coordinate care for patients recovering from stroke, brain, spinal cord, and other complex conditions. In this role, you’ll lead discharge planning, manage utilization review, and collaborate with an interdisciplinary team to ensure safe, timely transitions of care. You will educate patients and families, navigate insurance and community resources, and support evidence-based, patient-centered rehabilitation plans that restore function, independence, and quality of life in a mission-driven setting.
