Social Worker Position

Putnam Nursing & Rehabilitation Center is a 160-bed top rated skilled nursing facility located in Putnam County, New York and has been trusted by families to care for their loved ones. The social worker plays a vital role in supporting the emotional, psychosocial, and discharge planning needs of sub-acute residents.

• Conducting comprehensive psychosocial assessments upon admission and updating them regularly.
• Actively participating in interdisciplinary care plan meetings and coordinating services for safe and timely discharges.
• Providing counseling to residents and families, advocating for residents' rights, and promoting quality of life.
• Acting as a liaison, fostering communication among residents, families, staff, and outside providers.
• Involved in conflict resolution, maintaining compliant documentation, and helping residents adjust to facility life.
• Compiling social histories, preparing assessments and care plans, and revisiting them quarterly or as needed.
• Assisting residents and families in understanding care goals and limitations, and educating staff on the psychosocial aspects impacting residents.
• Addressing and resolving concerns using effective conflict resolution strategies.
• Maintaining accurate and timely documentation in compliance with regulations.
• Collaborating with clinical staff, case managers, and external providers.

Requirements:

• Minimum of a Bachelor's degree in Social Work or a related field, Master's degree in Social Work (MSW) preferred.
• LMSW licensure, preferred.
• Experience in sub-acute or skilled nursing settings, strongly preferred.
• Knowledge of Medicare/Medicaid guidelines.
• Strong organizational, communication, and interpersonal skills.
• Minimum of two years of experience in the social services field, preferably in a nursing facility, hospital or related agency.
• Pleasant personality and ability to work with residents, families, and staff from various departments.
• Proficient in Microsoft Office.
• Ability to work well under pressure and be flexible.
• Genuinely interested in helping ill, injured, disabled, elderly, emotionally upset, & sometimes challenging residents.
• Work well under pressure & willing to work harmoniously with all staff.
• Covid-19 vaccinations are encouraged but NOT MANDATED; we continue to offer COVID-19 vaccination based on eligibility guidance from CDC.
