Base Pay $105,000.00 - $150,000.00 / Year Job Category Medical Employee Type FT Exempt Required Degree Masters Manage Others No Key Responsibilities Provide comprehensive primary care, including assessment, diagnosis, treatment, and follow-up. Manage acute and chronic conditions, including diabetes, hypertension, cardiovascular disease, respiratory conditions, and other common primary care needs. Perform preventive health visits, screenings, immunizations, and patient education. Order and interpret laboratory, imaging, and other diagnostic studies. Prescribe medications and therapeutic interventions within scope of practice. Coordinate care with specialists, behavioral health providers, hospitals, and community resources. Maintain accurate and timely documentation in the electronic health record. Participate in quality improvement, population health, and clinical performance initiatives. Collaborate with the interdisciplinary care team to address medical and social factors affecting patient health. Support the health center's commitment to accessible, equitable, culturally responsive care. Requirements Qualifications Required: Graduate of an accredited Nurse Practitioner program. Current and unrestricted Nurse Practitioner license in New York State. Current national board certification appropriate to the patient population. Current BLS certification. DEA registration or eligibility, as applicable. Minimum 3 years of independent or advanced primary care clinical experience. Strong clinical judgment and demonstrated ability to manage medically and socially complex patients. Excellent communication, documentation, and interdisciplinary collaboration skills. Preferred: Previous experience in an FQHC, community health center, rural health center, or other safety-net setting . Experience caring for underserved or medically complex populations. Familiarity with population health, UDS measures, PCMH, value-based care, and quality improvement.
