Dialysis Social Worker - Patient Advocate & Care Lead

DaVit is looking for a Social Worker in San Antonio to join a trust-based, patient-centered team. You will support and advocate for dialysis patients, helping them navigate the physical, mental, emotional, and financial demands of End-Stage Renal Disease. Responsibilities include building relationships with patients, collaborating with a multidisciplinary team, and assisting patients in understanding insurance options.
