Key Responsibilities\n

• Conduct thorough psychosocial assessments by engaging with patients through interviews and observations, as well as reviewing medical records to understand their emotional, social, and environmental influences on recovery.

• Deliver empathetic counseling and support to patients and their families, addressing challenges related to adjustment and emotional well-being throughout the rehabilitation process.

• Collaborate closely with an interdisciplinary rehabilitation team to create tailored treatment plans that reflect the psychosocial needs and goals of each patient.

• Assist patients in navigating community resources, including support groups, vocational rehabilitation programs, and housing assistance to ensure comprehensive care.

• Provide educational resources to patients and families regarding available community resources and services that will facilitate successful transitions post-rehabilitation.

• Participate in discharge planning, ensuring smooth transitions for patients returning home or moving to other care settings, including facilitating any necessary follow-up services.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.
