Licensed Practical NurseThe Licensed Practical Nurse provides direct patient care under the direction of the RN professional. He/she assumes responsibility and accountability for patient care as a team member in order to achieve established outcomes.ResponsibilitiesA day in the life of a Licensed Practical Nurse at Hackensack Meridian Health includes:Performs non-interpretable data collection in an ongoing systematic manner, focusing on physiologic, psychosocial, and cognitive status.Provides hygienic care, assists with ambulation and meals as directed by the registered professional nurse.Positions patients, provides pre, post op and post mortem care as indicated.Uses the outcomes based practice model in the delivery of patient care.Provides a safe patient environment by answering call lights promptly, communicating with the RN immediately for any change in patient's condition.Identifies the needs of the patient population served and modifies and delivers care that is specific to those needs (i.e., age, culture, language, hearing and/or visually impaired, etc.). This process includes communicating with the patient, parent, and/or primary caregiver(s) at their level (developmental/age, educational, literacy, etc.). Other duties and/or projects as assigned.Adheres to HMH Organizational competencies and standards of behavior.Lifts a minimum of 5 lbs., pushes and pulls a minimum of 10 lbs. and stands a minimum of 6 hours a day.QualificationsEducation, Knowledge, Skills and Abilities Required:Graduate of an accredited program in Licensed Practical Nursing.Excellent written and verbal communication skills.Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.Licenses and Certifications Required:Licensed Practical Nurse.BLS AHA Basic Health Care Life Support HCP Certification within 60 days of entering position.
