Role Description

We are seeking a dynamic LVN Nurse Case Manager for a full-time field role working at ActivCare Living in San Diego, CA. This is an amazing opportunity to integrate with senior living, hospitals and skilled nursing facilities, and prospective residents in their homes. We are looking for talented team members who are called to serve a higher purpose.

Join our team to help onboard new residents needing a memory care community. The Nurse Case Manager will conduct in-person assessments of prospective residents evaluating physical condition, cognitive function, and care needs. This position will also network and build long-lasting relationships with physicians, discharge planners, and social workers.

The Nurse Case Manager will join our team of Nurse Case Managers and work in central and southern San Diego County (San Diego Metro to Chula Vista).

Responsibilities

• Perform comprehensive assessments of prospective residents to ActivCare Memory Care communities to help determine an appropriate level of care.
• Nurse case managers will travel to the resident whether at home, in hospital, skilled nursing, or other community.
• Interface with physicians, ActivCare staff, residents and families about resident care and condition.
• Interface regularly with ActivCare executive directors, program directors, nurses, and care staff.
• As their advocate, follow existing residents to hospitals and SNFs ensuring proper care and rehabilitation.
• Perform outreach to build relationships with discharge planners and social workers at SNFs and Hospital to seek prospective new residents.
• Refer residents to ancillary health care providers when appropriate.
• Compile and submit weekly assessment logs.

For the Nurse Case Manager position, we are looking for a strong leader who:

• Is currently licensed as an LVN in the State of California.
• Possesses strong critical thinking skills, be able to make independent decisions, and be a problem solver.
• Is knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines of long-term care.
• Is highly organized, efficient, and an excellent communicator.
• Has a valid California Driver's license.
• Has at least four (4) years’ nursing experience providing care to the elderly is preferred.
• Has been a case manager for at least two (2) years.

Estimated salary range is $42.00 - $50.00 depending upon experience.

Minimum Qualifications:

• Memory care: 2 years (Preferred)
• Nursing: 4 years (Required)
• Case management: 2 years (Required)

License/Certification:

• LVN (Required)

Ability to Relocate:

• San Diego, CA: Relocate before starting work (Required)

Willingness to travel:

• 75% (Preferred)
