Allied Technician - Physical Therapist

Physical Therapist (PT) - Home Health

We are seeking a dedicated and experienced Physical Therapist to join our Home Health team. The ideal candidate will have a minimum of one year of experience in home health care and be proficient in OASIS documentation.

Job Responsibilities

• Perform patient assessments and develop individualized treatment plans.
• Conduct regular evaluations to track patient progress and adjust treatment strategies as needed.
• Provide therapeutic interventions to enhance patient mobility, strength, and overall functional abilities.
• Educate patients and caregivers on exercises and strategies for home-based rehabilitation.
• Complete OASIS documentation and ensure compliance with agency policies.
• Manage scheduling and coordinate with other healthcare professionals.
• Monitor vital signs and assess pain, skin integrity, and overall health status.

Qualifications

• Must hold a current Physical Therapist license in Virginia.
• One year of experience in home health care required.
• Proficiency in OASIS documentation is essential.
• BLS certification is required; Manual Therapy certification is preferred.
• Ability to manage time effectively and document patient visits accurately.

Skills

• Assessment skills and pain assessment capabilities.
• Ability to identify environmental barriers and provide solutions.
• Knowledge of adaptive devices, orthopedics, and various patient populations.
• Experience in managing conditions such as COPD, CHF, and neurologic disorders.
• Competence in using equipment such as gait belts, Hoyer lifts, and specialty beds.

Work Environment

This position will primarily involve making home visits, with an expected workload of approximately 5 visits per day. Candidates must be comfortable working independently in various home settings.

Additional Information

• Orientation hours: 40
• Weekends may not be required, but flexibility for rescheduling may be needed.
• Business casual or solid color scrubs are acceptable for this position.
