WOUND CARE NURSE FRI-SUN

Position Summary: Job Summary The Wound Care Nurse is responsible for the assessment, treatment, and management of residents with acute and chronic wounds in a long-term care/nursing home setting. This role ensures evidence-based wound care practices, promotes optimal healing, prevents complications, and supports regulatory compliance through accurate documentation and staff education. Essential Duties and Responsibilities Assess, monitor, and document all resident wounds, including pressure injuries, surgical wounds, diabetic ulcers, and skin tears Develop, implement, and update individualized wound care treatment plans Perform wound treatments, dressing changes, and debridement as permitted by scope of practice Monitor wound healing progress and adjust care plans as needed Collaborate with physicians, nurse practitioners, dietitians, therapy, and interdisciplinary team members Ensure compliance with federal, state, and facility regulations related to wound care Provide education and guidance to nursing staff on wound prevention, treatment protocols, and skin integrity Participate in pressure injury prevention programs and quality improvement initiatives Identify residents at risk for skin breakdown and implement preventative interventions Maintain accurate, timely documentation in the medical record Order wound care supplies and ensure appropriate use Participate in care plan meetings, audits, and surveys as needed Qualifications Current, unencumbered RN or LPN/LVN license in the state of employment Minimum of 1–2 years of nursing experience; long-term care experience preferred Wound care certification (WCC, WOCN, CWCN) preferred or willingness to obtain Strong knowledge of wound assessment, staging, and treatment modalities Familiarity with CMS guidelines and long-term care regulations Excellent organizational, communication, and documentation skills Physical Requirements Ability to stand, walk, bend, and lift residents or equipment as needed Manual dexterity to perform wound treatments and dressing changes Ability to work in a fast-paced clinical environment #J-18808-Ljbffr Abbington-Village-Nursing-and-Rehabilitation-Cente
