Licensed Clinical Social Worker (LCSW)

Role and Responsibilities Provide discharge planning and psychosocial interventions through assessment, advocacy, referral, and interdisciplinary collaboration. Prepare social histories and assess patients' social, emotional, psychological, and financial needs. Manage policies for setting and evaluating residents’ goals related to social functioning. Offer social casework to individuals and families receiving healthcare services, aligning with physicians' plans. Assist healthcare providers in understanding social and emotional factors affecting patients. Document services per regulations, standards, and third-party payer guidelines. Qualifications & Skills Licensed Clinical Social Worker (LCSW) credential required. Minimum of 2 years of relevant work experience. Strong decision-making, communication, and interpersonal skills. Preferably hold a Master’s degree from a CSWE-accredited program. Compassionate, attentive to detail, and knowledgeable of procedures and patient safety. Key Attributes & Requirements Ability to work independently and make decisions confidently. Excellent communication skills with staff, physicians, patients, and families. Willingness to work in a rotating 5x8 shift schedule.
