Join an established and well-respected team of 8 physicians and 3 APP's Hybrid role This position will combine virtual and in-person work, with focus on flexibility based department needs. The APP will provide virtual support for Bartlett and neighboring offices with limited access. On average, the expectation is four days of remote work and one day in person Call structure: office call, one weekend every six weeks Practice setting: Outpatient Full-spectrum Family Medicine (kids-adults) High quality service in one convenient location, including primary care and specialists Lab testing and imaging services on site Excellent office support staff Board Certified Nurse Practitioner or Physician Assistant Eligible for state medical license and DEA/Controlled Substance registration About the Facility 387 bed hospital Level II trauma Integrated referral network Part of Ascension, a national nonprofit Catholic health system Why Physicians Choose Ascension Physician-led, collaborative culture Long-term practice stability National network with local autonomy Mission-centered care rooted in service What minimum qualifications you'll need Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Nurse Practitioner credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required. Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred. Registered Nurse preferred.
