Care Facilitator

Care Facilitator Serve as a patient’s first point of contact, delivering exceptional customer service and administrative support at the clinic. Responsibilities Interact with patients, greet, check‑in, check‑out, and schedule appointments. Maintain lobby organization and cleanliness. Gate‑keep PCP and specialist schedules, ensuring high‑priority patients are scheduled every 13 days. Use computer systems to add new patient information, print forms, schedule appointments, respond to emails, and generate new patient packets. Verify and document patient contact, emergency contact, and insurance information; collect copays. Conduct “love” calls, mail birthday and greeting cards to patients. Answer phones, take detailed messages, and route them appropriately. Communicate clinic personnel information to patients and others. Assist with scheduling follow‑up and specialist referral appointments. Escort patients to exam rooms and ensure comfort before departure. Act as HEDIS champion for the PCP, providing visibility of HEDIS needs. Report hospitalizations, HITS census, and ENS Ping data. Present customer service surveys at checkout and handle escalations. Participate in daily and weekly huddles, providing patient details. Regulate and monitor transportation, including driver lists, medication deliveries, grocery deliveries, driver behavior, and coordinating Uber/Lyft rides for patients. Maintain confidentiality of patient information and records. Review patient appointment information for completeness and accuracy. Transmit correspondence by mail, email, or fax. Perform other duties as assigned. Qualifications Exceptional customer service skills and passion for serving others. Working knowledge of medical insurance and/or electronic medical record (EMR) systems. Knowledge of ChenMed medical products, services, standards, policies, and procedures. Skill in operating phones, personal computers, software, and basic IT systems. Professional and courteous communication with employees, patients, and others. Detail‑oriented with accuracy in reports and data. Strong verbal and written communication, listening, and positive attitude. Proficient in Microsoft Office Suite (Excel, Word, Outlook). Willingness to travel locally and regionally up to 10% of the time. Spoken and written fluency in English. Flexible to work evenings, weekends, and/or holidays as needed. Education and Experience High school diploma or equivalent education required. Graduation from a nationally accredited Medical Assistant program preferred. Minimum of 1 year of work experience in a medical clinic or similar environment required. BLS for Healthcare Providers preferred. Pay Range $14.30 – $20.42 hourly. #J-18808-Ljbffr ChenMed LLC
