SSM Health Good Samaritan Hospital in Mount Vernon, IL is seeking a Registered Nurse for case management. The role involves discharge planning with an interdisciplinary team to ensure timely transitions to the next level of care and to improve patient outcomes. Ideal candidates have 3–5 years of medical/surgical or ICU nursing experience, prior case management experience preferred, and strong communication and detail orientation.
