Minimum Requirements

Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
Active Michigan Physical Therapy license required to start the assignment.
BLS certification from AHA or ARC may be required.
Home Health experience preferred; open to candidates new to the home health setting.
Experience with Home Care Home Base EMR preferred.
Productivity expectation: ability to meet approximately 30 points per week.
Willingness to travel within approximately a one-hour radius of the hub city to cover patient referrals.
Interview via Teams or video call.

Shift: Days.
Shift hours per week: 36.
Weekend rotation: approximately one weekend per month, as needed.
Mileage reimbursement: IRS standard mileage rate.
On call: None.
