Responsibilities
- Deliver routine hospice nursing care to patients in outpatient facilities and at home, typically 4–5 visits per day
- Travel 30–50 miles daily to reach first patient; follow-up visits are in nearby areas
- Assess patient conditions, manage symptoms, administer medications, and adjust care plans accordingly
- Develop and document individualized care plans; coordinate with the interdisciplinary team and physicians
- Educate patients and families on end-of-life care, pain management, and care expectations
- Maintain accurate documentation, ensure safety and infection control, and respond to changes in patient status

Requirements
- Active Massachusetts RN license required at time of submission
- BLS certification
- Experience in hospice or outpatient/home hospice nursing
- Ability to travel 30–50 miles daily; reliable transportation
- Availability for day shift work (8:00 a.m.–4:30 p.m.), Monday–Friday
- Must reside outside 50 miles from the assignment facility; local candidates will not be considered
