SLP - Speech Therapist

Job Title: SLP - Speech Therapist Duties Job Responsibilities: Performs Patient assessments: Accurately assesses patient's appropriateness for home care, learning needs and progress toward goals. Collaborates with patient, family, and physician to develop the plan of care for the patient. Performs baseline assessment of patient swallowing, communication, and cognitive abilities as indicated. Maintains safe environment for patients and co‑workers: Assess social environment and adjust or provide education as needed. Assess for potential clinical risks and adapt program as indicated. Follows universal infection control measures. Implements treatment program to achieve designated goals: Administer occupational therapy services per physician orders. Instruct in communication and safe swallow, cognitive training as indicated. Develop and instruct in therapeutic exercise program. Implement program to restore function, and achieve maximum functional status. Provides discharge planning and referral to resources as needed. Communicate with physician, nurse, and other clinical disciplines as necessary. Develops/implements/evaluates plan of care based on evidence‑based knowledge. Revise care plan as indicated within physician orders. Uses age‑appropriate teaching methods for patients and families: Evaluates teaching plan and revises as necessary. Documents patient education tools utilized. Demonstrates compliance with agency policies: Assumes responsibility for Case Management. Demonstrates knowledge of home care regulations and insurance reimbursement guidelines. Assures compliance with regulatory requirements. Follows agency documentation policies. Shows evidence of interdisciplinary communication. Ensures appropriate utilization of services. Follows scheduling process. Maintains visit productivity standards. Participates in Team and IDT meetings. Prioritizes schedule to accommodate patient's needs. Travels to patient homes and facilities as indicated. Complies with paid time off request process and weekend/holiday requirements. Assists with growing agency business: Provides orientation to new employees. Accepts other assignments as requested. Cross trains to learn new skills. Use of personal vehicle is required for home visits. Skills Required Skills & Experience: Minimum of one (1) year of experience in acute, sub‑acute or home care setting. Good customer service and communication skills. Education Graduate of an accredited speech and language program with a minimum of a master's degree. Certifications & Licensure Current NJ ST License. Shift Mon‑Fri 8a‑4pm, No Weekends #J-18808-Ljbffr
