Days/Hours: Monday - Friday, 8hr day shift
As a hybrid Kidney/Pancreas Transplant Pharmacist at Cedars-Sinai Medical Center in Los Angeles, California will maintain the quality and support of highly immunocompromised patients requiring high level of specialized services. The Kidney/Pancreas Transplant Pharmacist will help manage nephrology consults, pre-transplant infusions for highly sensitized patients, post-transplant infusion patients, and cover in-patient services including managing non-formulary requests. The Pharmacist will be responsible for entering therapy plans, conduct chart reviews, compose letters of medical necessity as well as participate in peer-to-peer reviews for denied insurance claims. The Pharmacist will also participate with in-patient rounding which would include discharge teachings, medication reconciliation, ordering and coordinating last minute medication changes prior to discharge.
Responsibilities
- Performs the duties of a pharmacist to support patient care needs through coordination of the medication use process, including accurate and timely review and distribution of medications. Ensures safe and effective use of pharmacy automation and technology.
- Assures appropriateness of drug selection, dose, route, duration, frequency and timing of medications by providing recommendations to prescribers on appropriate dosage adjustments, drug therapy alternatives or discontinuation of medications. Ensures safe care through pharmacist-driven protocols and as outlined in the Clinical Service Plan.
- Appropriately receives ordered medications and supplies and ensures proper storage and handling and assists in maintaining proper inventory records. Performs labeling, packaging, obtaining medication and getting it ready for distribution and utilizes dispense prep to accurately prepare doses as required.
- Responsible for coordination of sterile/non-sterile compounded medications, including accurate, timely preparation and delivery and management of controlled substances (CS) inventory, such as returns, outdates, receipts and destruction, as well as drug shortages.
- Ensures compliance with controlled substances (CS) inventory requirements, such as returns, outdates, receipts and destruction.
- Documents clinical interventions, including near miss prescribing events and recommendations for safe and effective medication management.
- Participates in transition of care programs by performing guideline direct medication therapy reviews during transitions of care, such as admission medication history reviews, discharge medication list reviews and post discharge case management; and as defined by the department's transitions of care program in a timely manner consistent with national standards, if applicable.
- Actively supervises technicians/interns and coordinates technician/intern functions.
- Provides medication education to patients/family for priority medications per departmental initiatives or if patient consultation is requested/warranted.
- Actively identifies methods and approaches to simplify work processes, decrease medication wastage, and increase the quality of services provided.
- Acts as a liaison in medication-related problem solving for patient care units, including but not limited to narcotic discrepancies, automated dispensing cabinet/cart discrepancies and drug storage issues.
- Demonstrates change management/engagement, such as actively supporting organization and departmental initiatives and exhibits flexibility and adaptability to changes in workflows, responsibilities, work assignments, procedures, and new work situations. Stays abreast with the medical center and departmental information, such as with emails, minutes and other department sources.
- Participates in the education and training of pharmacy staff, residents, interns, students, and other healthcare professionals.
- Adheres to state and federal regulations, professional and regulatory agency standards, and assures required documentation is complete, accurate and timely.
Qualifications
Educational Requirements:
- Pharm.D. or equivalent degree from an accredited school of pharmacy.
- Completion of a PGY1 and/or PGY2 residency or equivalent practice experience.
License/Certification/Registration Requirements:
- Current pharmacist licensure in the State of California.
- BLS certification as outlined in the Pharmacist Responsibilities in Cardiopulmonary Resuscitation (C.P.R.) and Code Blue Procedure: Pharmacy Policy and Procedure

- Ability to teach and serve as preceptor for training programs.
- Ability to prepare drugs needed for immediate administration.
- Ability to provide recommendations and monitor the use of drugs.
- Knowledge of laws, regulations and standards relative to pharmacy practice.
- Ability to adapt to change.
- Ability to prioritize workflow.
- Ability to evaluate medication regimens to meet the therapeutic needs of patients.
- Ability to learn and adapt to the use of automated information systems in daily practice.
- Demonstrated competency in techniques of pharmaceutical calculations, compounding and pharmacokinetics.
- Possess patient-centered communication skills.
- Possess computer skills necessary to perform job functions.
