Registered Nurse - Case Manager

Base pay range $55.00/hr - $60.00/hr Duties Perform care coordination and discharge planning for patients. Utilize InterQual Criteria for admission criteria in the Emergency Department. Participate in the development of treatment plans and family meetings. Work with interdisciplinary support including Social Services and Transportation. Arrange Durable Medical Equipment (DME), Skilled Nursing Facility (SNF), and ambulance transport independently. Manage patient diagnoses, including drug/alcohol abuse treatment, homelessness, mental health crises, and more. Document patient care using computerized systems. Skills Proficient in Microsoft applications. Experience with HMOs, IPAs, and related healthcare management organizations. Competence in arranging patient transport and equipment independently. Experience Minimum of 2 years of acute care hospital case management experience, specifically in care coordination and discharge planning. Experience with Epic charting system is required. Travel experience is required, with first‑time travelers accepted. Must have a CA state license or be licensed upon the start date. Experience with CMS and DRG is preferred.
