Medical Social Worker (MSW) - Home Health

Seeking a compassionate and experienced MedicalSocial Worker (MSW) to provide social work services to patients receivingcare in their homes. The Medical Social Worker will collaborate with patients,families, caregivers, and members of the healthcare team to addresspsychosocial needs and support individualized patient care plans.

This position will serve patients in the Kansas City,Missouri area and surrounding communities . The ideal candidate iscompassionate, organized, self-motivated, and able to effectively manage aschedule while working independently in a home health setting.

Key Responsibilities

• Assesspatients’ psychosocial needs and identify appropriate resources andsupport services.
• Collaboratewith the interdisciplinary care team to develop and implementindividualized patient care plans.
• Providecounseling, emotional support, and guidance to patients and theirfamilies.
• Assistpatients and caregivers with navigating community resources and availableservices.
• Helpaddress social, emotional, financial, environmental, and other factorsthat may affect patient care and well-being.
• Coordinatewith healthcare providers and community agencies as appropriate.
• Maintainaccurate and timely clinical documentation in an Electronic Medical Record(EMR) system.
• Completerequired assessments, progress notes, and other documentation withinestablished deadlines.
• Managepatient visits and daily responsibilities independently.
• Travellocally to patients’ homes throughout the Kansas City area and surroundingcommunities.
• Maintaincompliance with applicable professional, clinical, and regulatoryrequirements.

Required Qualifications

• Master’sdegree in Social Work (MSW) or higher from a graduate programaccredited by the Council on Social Work Education or an appropriatelyaccredited institution.
• 1–2years of post-graduate social work experience in a healthcare setting.
• Currentand valid Medical Social Worker license in the state of Missouri (MO) and in good standing.
• Atleast 1 year of experience using an Electronic Medical Record (EMR) system, including TruChart EHR or another EMR platform.
• Currentand valid driver’s license .
• Reliablemode of transportation and current auto insurance.
• Abilityto travel locally to patient residences, with approximately 20% of theworkday spent driving .
• Strongorganizational and time-management skills.
• Abilityto independently manage a patient schedule and prioritizeresponsibilities.
• Abilityto consistently meet required clinical documentation and chartingdeadlines.
• Demonstratedproficiency with tablets, smartphones, and EMR software.
• Strongcommunication, interpersonal, and patient advocacy skills.
• Commitmentto providing compassionate, patient-centered care.

Travel

• Localtravel to patients’ residential homes is required.
• Theposition primarily serves Kansas City, MO and surrounding areas .
• Approximately 20% of the workday may involve driving to and from patient locations.

Candidate Requirements

• Musthave a valid Missouri Medical Social Worker license.
• Musthave a valid driver’s license, reliable transportation, and current autoinsurance.
• Mustbe comfortable with the required local travel.
• Candidatesmust not have applied to or worked for Residential Healthcare Groupwithin the past 6 months .
