Deliver vital rehabilitation services to patients in the comfort of their own homes as a Home Health Physical Therapist (PT) on a full-time, 13-week contract. This dynamic role empowers you to support individuals in regaining mobility, function, and independence making a real difference in their daily lives. Youll conduct comprehensive assessments, design and implement individualized treatment interventions, and serve as a pivotal member of an interdisciplinary healthcare team.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active, unrestricted Physical Therapist (PT) license in the state of practice
• Minimum of one year of recent Home Health Physical Therapy experience (field-based experience preferred)
• Proficient with Home Care Home Base (HCHB) documentation platform preferred
• Required experience using the OASIS assessment tool
• Reliable transportation, valid drivers license, and appropriate auto insurance
• Strong clinical assessment, organizational, and communication skills
• Ability to work independently and effectively manage a field-based schedule

Primary Duties:

• Perform thorough physical therapy evaluations, including OASIS assessments at various care milestones
• Develop and implement care plans to address functional mobility, pain management, safety, and fall prevention
• Deliver skilled interventions: therapeutic exercise, neuromuscular re-education, transfer and gait training
• Communicate patient progress and changes with physicians and healthcare team members
• Instruct patients and caregivers on exercises, use of assistive devices, and safe home routines
• Maintain accurate, real-time documentation in HCHB
• Participate in case conferences, ongoing education, and interdisciplinary collaboration
• Travel daily to patient homes within their assigned territory (mileage reimbursement provided)

$2,602 - $2,753/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
